- Appointment and health information. Information you share in the course of care — medical history, treatment records, photographs — is collected and retained as part of your patient record, separately from this website.
- Technical information. Our web host records standard server data such as IP address, browser type and pages visited, for security and performance purposes. We also use Google Analytics to measure how the website is used (see “Cookies and analytics” below).
Online booking
Appointment booking on this site is provided by Boulevard, a third-party scheduling platform. When you open the booking overlay, any information you enter is submitted to and processed by Boulevard under their own privacy terms, and shared with us so we can manage your appointment.
How we use your information

Protected health information
Health information created or maintained in the course of your care is Protected Health Information (PHI) and is safeguarded under the Health Insurance Portability and Accountability Act (HIPAA) and applicable Michigan state law. This website policy does not replace our Notice of Privacy Practices, which is provided to patients directly. Please ask us for a copy.
Where we use third-party providers that create, receive, maintain or transmit PHI on our behalf — including our scheduling and client management platform — we do so under a signed Business Associate Agreement requiring them to safeguard that information to the same standard.
Photographs and consent
We do not publish patient photographs, including before-and-after images, without separate written consent. Consent is specific, voluntary and may be withdrawn at any time by contacting us, after which we will remove the images from our website and marketing materials going forward.
Sharing your information
We do not sell your personal information. We share it only with service providers who help us operate the practice — such as our scheduling platform and electronic records provider — and where required by law or to protect health and safety.
Cookies and analytics
We use Google Analytics, a service provided by Google, to understand how visitors use this website — for example, which pages are viewed, how visitors arrive at the site, and the general type of device and browser used. Google Analytics sets cookies in your browser to do this. We use this information only in aggregate, to improve the website. We do not use Google Analytics for advertising, and we do not use it to collect the information you enter when booking an appointment.
The Boulevard booking overlay may set cookies that are necessary for online booking to work. The fonts on this website are hosted on our own server and do not set cookies.
You can opt out of Google Analytics by installing Google’s opt-out browser add-on, available at tools.google.com/dlpage/gaoptout, or by blocking or deleting cookies in your browser settings. Blocking cookies may stop some features, such as online booking, from working as intended.
Your choices
You may ask us to access, correct or delete the personal information we hold about you, subject to the record-retention obligations that apply to medical records. To make a request, contact us using the details below.
Children
This website is not directed to children, and we do not knowingly collect information from children through it.
